Coach Registration
All Head Coaches and Assistant Coaches Must Register
A Complete Coach Registration Package Includes:
1. Coach Registration Form
• This form is submitted online with an electronic signature, after which you will receive a confirmation receipt.
2. Photo (.jpeg or similar)
• The image should clearly show your face, similar to a driver’s license photo.
• Photos with hats or sunglasses will be rejected.
3. Team Seeding Form
• Head Coaches Only
• Coaches in the Under-6 age group do not need to complete the Seeding Form.
• Fill it out electronically, then save and e-mail.
Please e-mail your registration confirmation number, photo, and seeding form to the Registrar. If necessary, you can also email scanned versions of these documents.

Coaching Clinics
These clinics are not required to coach recreational soccer, but are incredibly useful and fun. Using US Soccer guidelines these clinics will provide the
framework and skills needed to run productive practices, help develop your players, and provide you with the tools to
successfully manage your team. Beginning and unlicensed coaches begin with
the F clinic. Please refer the to SYSL website for
upcoming clinics in our area.
Injury Reports
CYSA requires that coaches fill out and submit an Injury Case Report
whenever a serious injury occurs during a soccer related event. Please help protect yourself, your players, and the league by filling this form out when needed.
